It can occur if the receiver has a 'power save" mode like my 91AD has.
It switches the receiver part on and off quickly with an ever
decreasing duty cycle (starts at about 50% and several times a second,
down to about 16% on-time after couple of minutes.
What happens is that the call ID information is sent at the very start
of the transmission. If the receiver happens to be in the "off" part
of the cycle when the beginning of the signal is received, then it
will not receive the caller ID, but, after a second or so - short
enough for us not to notice the pause, but certainly enough for the
data to be missed - the receiver turns on again and picks up the
signal. It then records a blank received call entry.
The manual even says that "This is normal, not a malfunction, because
the header of the call sign cannot be detected during the power save."
It then recommends turning the power save feature off if this is an
issue.
