The system's changed since I got mine, the process now is you go to 
the WIA web site and read up on the process there.  You need to 
submit all your details to the WIA, and then it will proceed from 
there, from what I understand.

Cost?  The repeater licence is the same cost as a normal licence.  I 
believe there is a WIA callsign assignment charge as well, you'd have 
to look that up.

Anyway, here's the derails.
